With $100M from CVS, Carbon Health Launches New Virtual Remote Monitoring Program

Carbon Health, which received a $100 million injection from CVS, has begun rolling out a program that remotely monitors and provides virtual patient care in Massachusetts.

The Connective Care initiative sets goals for patients, which are then monitored using data that is self-reported or collected from devices such as connected blood pressure cuffs. The data is integrated into Carbon Health’s own EHR. Patients can access primary care providers supported by behavioral health and chronic disease specialists in person or virtually.

The program began in the first week of January and will continue throughout the state this year.
